Brief Overview
Chronic Disease Management is an additional module that is enabled through a provider's Consult Settings. This module allows a provider to track the progress of chronic diseases such as asthma, arthritis, diabetes, depression and more.
Before you begin the instructions in this guide, you will need to ensure that the provider has the Chronic Disease Management module enabled. The instructions for this can be found in this article.
Short Version
- Enable Chronic Disease Management in the provider's settings.
- Open the Chronic Disease Management tab when within a Current Consultation.
- Select the condition to manage from the Disease menu.
- Choose a Type (if appropriate).
- Record all necessary measurements.
- Click Finish Consult.

Instructions
Chronic Disease Management
1. Open the Chronic Disease Management tab when within a Current Consultation.
2. Select a Disease from the drop-down menu.
Note: If the Disease you wish to manage is missing from this menu, review the diseases you have selected to manage in your Consult Settings.
3. Select a Type/Stage from the drop-down menu (if relevant).
4. The fields available to you in the window will change depending on the Disease and Type you have selected. Complete the fields as required.
Notes: The following resources may be helpful for you to read before you begin, if you are yet to learn how to record vitals, investigations, view current medications and prescribe, or review results.
Coronary Heart Disease
Types: Systolic Heart Failure/Diastolic Heart Failure/Congestive Heart Failure/Left Ventricle Failure/Right Ventricle Failure

5. You may then add any other items to the clinical record as required, and click Finish Consult.
Risk Assessments (Diabetes)
1. Open the Risk Assessment tab from within the Clinical File, or from the Chronic Disease Management module.
2. Click the Plus + icon to add a new Risk Assessment.
3. Complete the assessment by selecting from the options provided. The Risk Assessment points will be tallied, and displayed at the bottom of the window.
4. Click Save.
5. The Risk Assessment will then be displayed within the Results section.
6. If you would like to review a result or make any adjustments, click on the Edit icon beside the report.
7. You may then add any other items to the clinical record as required, and click Finish Consult.
